This Privacy Policy explains how Upper Valley Direct Primary Care, PLLC. ("we," "us," "our") collects, uses, and protects information from visitors to our website at uvdpc.com (the "Site").

This policy covers website visitor information only. If you are a patient or member of the practice, the way we handle your Protected Health Information (PHI) under clinical care is governed separately by our HIPAA Notice of Privacy Practices, available upon request.

1. Information We Collect

a. Information collected automatically

When you visit the Site, our website host, Squarespace, automatically collects some technical information to operate, secure, and (if enabled) measure traffic on the Site. This may include:

  • IP address

  • Browser and device type

  • Pages viewed and links clicked

  • Approximate location (derived from IP address)

  • Referring website / how you found us

We do not control the specific technical mechanism Squarespace uses to collect this.

b. Information you choose to provide

The Site itself does not contain any forms, quizzes, or fields asking you to enter information. If you are interested in joining the practice, we direct you to an external Google Form, hosted by Google, where you may voluntarily provide:

  • Name

  • Email address and/or phone number

  • We use this information solely to contact you by email about membership in the practice. Submitting the Google Form is governed by Google's own privacy policy in addition to this one, since Google hosts and processes that form on our behalf.

  • We do not use this contact information for anything other than responding to your inquiry, unless you separately become a patient, at which point your information is handled under our HIPAA Notice of Privacy Practices.

2. How We Use Information

We use the information described above to:

  • Operate, maintain, and secure the Site

  • Understand how visitors use the Site so we can improve it (only if site analytics are enabled)

  • Respond to inquiries submitted through our Google Form

  • Comply with legal obligations

We do not sell visitor information, and we do not use advertising pixels (such as a Meta/Facebook Pixel or Google Ads remarketing tag) on the Site.

3. Third-Party Service Providers

You can decline to submit the Google Form; doing so will not affect your ability to browse the Site.

You can control cookies through your browser settings (see Cookie Policy).

You may contact us at any time to ask what information we hold about you or to request that we delete it, to the extent we are not required to retain it by law.

7. Children's Privacy

This Site is not directed to children under 13, and we do not knowingly collect information from children through the Site.
